2013 CMO Request Form

The Colorado Missions Offering - Nicy Murphy Offering is designed to assist Colorado Baptists' churches with projects, special events, activities and resources to help fulfill the great commission in our state. The purpose of this offering is not to supplement operating budgets, but to specifically assist missions and ministries to reach out in their community with the love of Christ.

To request assistance from the CMO, the congregation must be a cooperating Southern Baptist congregation, contribute regularly to the Cooperative Program and must have contributed to the CMO the previous year (2012). Contributions must be received by December 31, 2012.

Affiliate with your association & state convention

If your church has not already petitioned your association and the Colorado Baptist Convention for affilitation, now would be a great time.

Please note that your local association and the state convention are two seperate organizations and will have different ways to affiliate.

Do you want to discover God’s call on your life? This booklet can help you. The North American Mission Board hopes that you will take time to prayerfully go through these pre-assessment tools. We believe this is a great first step in your journey into church planting. If you are married, we encourage you to get a copy of this booklet for your spouse. These tools can help you understand your roles in a new church.

Your journey begins by discovering God’s call on your life. This is something that you should take seriously. Experienced church planters tell us that one’s call is vitally important to their ministry. God’s call on your life to a specific place at a specific time will keep you there when you otherwise might want to give up.

PIF - Personal Information Form (NAMB)

This form is used to share information about individuals who are applying for or receiving any type of NAMB funding.

After completing your application form (PIF), please send Reference Form to all your references listed on Page 4.

Remittance Form for Cooperative Giving

Summer Missionaries

If your church or ministry is seeking a summer or semester student missionary for 2011, then it is necessary to fill out the attached request form* and return to Wade Pacheco.  This request form is to seek college students willing to serve 10 weeks throughout the summer or during a typical college semester (a few months either Fall or Spring semesters).  These students can be utilized by your church or ministry to help you with community evangelistic outreach projects.  Please see the attachment below for more information.
